Job summary
The post holder will act as a Parkinson's Disease Nurse practitioner through strong clinical leadership and effective management and will support primary care services within Neurology and Elderly Care Service inMaidstone & Tunbridge Wells Hospital NHS Trust and Kent Community Health Foundation NHS Trust to promoteand deliver nurse excellence for adults with Parkinson's Disease. This role requires a good level ofcompetence, nursing expertise and evidence based clinical care and decision-making be able to work autonomously in managing a defined caseload of adults with Parkinson's under thesupervision of Band 7 PD Nurse Specialist. The post holder will be involved in direct patient care for adults withParkinson's Disease from initial diagnosis through to the palliative stages of the illness, by providing advice,assessment and medication management to patients, their families/carers and post holder will be responsible for delivering nurse led clinics on a 1:1 basis with patients as well asproviding community visits and supporting educational patient groups for patients who are newly diagnosedwith Parkinson's and potentially offer an in-reach service to local hospitals to prevent hospital admissions andcoordinate better care packages upon discharge where needed.
Main duties of the job
To assist the Community Parkinson's Disease Specialist Nurse in the provision of the specialist nursingservice for all adult patients with Parkinson's Disease across the West Kent locality.
1. To manage a caseload of clients requiring specialist Parkinson's Disease nursing, within the community.
2. Undertake assessment and treatment of clients within both clinic and community setting, who havecomplex / and or chronic presentation requiring specialist skills; and determine profession specific andmultidisciplinary rehabilitation interventions.
3. To provide advice, education and support to patients, their carers and healthcare professionals, and assist incoordinating treatment and care.
4. To work across professional, departmental and Trust boundaries to support the delivery of a patient-centred seamless service.
5. To work in close collaboration with the Community Neuro & Specialist Services, delivering and evaluatingcare pathways for clients with long term neurological conditions.
This is a full-time role and a car driver and access to own vehicle is essential to the role.
