The specific purpose is to deliver care in a collaborative manner that reflects the principles of AOT working, particularly aspects relating to recovery and social inclusion. The Cheltenham, Tewkesbury and North Cotswolds Assertive Outreach Team provides comprehensive mental health and social care to working age adults in the Gloucester locality and the Forest of Dean. Main duties of the job This is an exciting opportunity for a community mental health nurse who has some experience of working with adults of Working Age and caring for individuals with Enduring Mental Health problems. The post will include care coordinating a designated caseload, working within CPA Policy and associated countywide risk procedures, travelling independently, use of IT skills and a working knowledge of legislation regarding safeguarding vulnerable adults, the MHA and MCA.
