Job summary We have an exciting opportunity for an experiencedclinical pharmacist tojoin our busy practice,on a Fixed term contract - Pilot initiative 24hours per week You will be workingalong-side our multi-disciplinary team to deliver the best healthcare service for our patients. Main duties of the job As a Pharmacist you will be responsible for working alongside GPs and other member of the clinical team dealing with patients'clinical medication reviews, including those with complex polypharmacy. The post holder will provide primary support to general practice staff with regards to prescription and medication queries. They will help support the repeat prescription system; deal with acute prescription requests. Furthermore, they will promote systems for safer prescribing and provide expertise in clinical medicines advice. About us Our practice is based in the park hill area of Sheffield. We have 4 GP Partners and 1 Salaried GP's. Our nursing team consists of 1 ANP, 1 pharmacist, 2 Practice Nurses and 1 Health Care Assistants and care coordinator. We are part of the GPA1 PCN which consists of the 5 local practices which share variety of services including a physiotherapist and Mental Health Team.
