Band 6 - Social Work Practitioner Intensive Support Team, South Staffordshire
We are seeking an enthusiastic and well-motivated Social Work Practitioner to work as part of our multi-disciplinary Intensive Support Team (IST), and share the delivery of social care intervention with their Social Work Practitioner colleague.
The IST provides services to adults with Learning Disabilities and behaviour that challenges. The team is based in Stafford and covers the South Staffordshire area. The post holder will engage with service-users in a variety of settings. The successful applicant will be joining a team with an outstanding value base, and high expectations of supporting good quality of life outcomes for the people they serve. Previous experience of working with people with learning disabilities is essential, preferably as part of a community team.
Main duties of the job
The post holder will have the opportunity to work jointly with other members of the IST within a shared caseload of people who require intensive support. This is a unique opportunity for an experienced Social Worker to be able to work in a focused way with people with a learning disability and their carers. The post holder will be expected to provide specialist social care input to contribute to the assessment and support planning for service users with a learning disability who experience complex behaviours and/or mental health needs. This will include working collaboratively with other disciplines and agencies in order to provide the highest quality service to service users.
The Intensive Support Team consists of nurses, psychologists, occupational therapists, Speech and Language Therapists and assistant practitioners and works closely with the local Community Learning Disability Team, and Dynamic Support Register Service.

Job responsibilities
KEY DUTIES AND RESPONSIBILITIES:
* To promote the rights of individuals receiving a service to be treated with dignity and respect.
* To promote involvement in all aspects of the service individuals receive valuing choice and promoting independence where ever possible.
* To maintain strong networks and work in partnership with the local authority Learning Disability Team.
* To provide a comprehensive social work service including specialist social work assessment.
* Active contribution of the social care perspective to risk assessment, care planning and liaising with current providers, carers and family.
* To liaise effectively with statutory and non-statutory bodies in formulating comprehensive assessments of need and in identifying appropriate resources for service users to include liaising with current providers, carers and family.
* In accordance with local policy and good practice guidance, to manage issues of confidentiality in relation to all service-users, including those with high media profile.
* To ensure a high standard of record keeping and the integrity of files.
* To prepare full and regular written reports for MDT review meetings etc. and to attend such meetings as appropriate for all service-users within allocated case-load.
* To prepare comprehensive statutory reports for Mental Health Review Tribunals and Hospital Managers Hearings if required and to attend and give evidence at such meetings.
* To offer considered recommendations to these bodies.
* To provide written reports for Crown or Magistrates Courts and to attend and give verbal evidence as required.
* To be proactive in regularly meeting and reviewing all allocated service-users.
* To initiate contact with families and informal carers and support the undertaking of formal Carers Assessments in accordance with statutory legislation (where applicable).

Person Specification
Qualifications & Training
* A recognised Social Work qualification, CSS, CQSW, DipSW, etc.
* Current Professional Social Work registration (HCPC).
* Evidence of current professional development.
Experience
* Substantial experience of working as a qualified social worker.
* Experience of working with people with a significant learning disability who present with behaviours that challenge.
* Evidence of working within a multi-disciplinary team.
Skills, Knowledge & Abilities
* Good written and verbal communication (English) skills. High level presentation skills in formal settings.
* Working knowledge of relevant legislation including those relating to Care Act 2014; Mental Capacity Act 1995, NHS and Community Care Act 1990, and the Mental Health Acts 1983 & 2007 (including DOLS, IMHA, IMCA, & Court of Protection).
* Ability to work with a customer focus, respecting individual's rights, promoting their independence and maintain their dignity and personal respect.
Personal Attributes
* To be able to travel independently as required over a large geographical area to fulfil duties of the role.

Salary: £37,338 to £44,962 a year (pro-rata)
