The Mental Health Social Care team is part of Rotherham Metropolitan Borough Council’s Adult’s and Community Wellbeing Offer. The team comprises four subsections supporting adults with a functional diagnosis of mental health.
The AMHP Team focuses on referrals under the pathway of the Mental Health Act 1983 (Amended 2007). The team works across all age groups and with individuals experiencing both organic and functional mental ill health.
The Mental Health front door team is a duty team that responds to the most urgent cases needing support under the Care Act 2014. This service also includes the Mental Health Enablement Pathway, ensuring the duties of The Care Act are adhered to promote and achieve the prevention and wellbeing principle.
There are then two community teams, ‘North’ and ‘South’, that manage all long-term Care Act mental health support and reviews.
The team provides proportionate and targeted support to people at the right time to ensure maximum independence. The service is focused on recovery, reablement, empowerment, and social inclusion, emphasizing prevention and early intervention, managing demand through proactively engaging and supporting people with functional mental health needs that significantly impact their lives.
This is an excellent opportunity to develop work with individuals to achieve their goals/outcomes and promote independence and autonomy using a strength-based, person-centred approach. You will be working as part of one of the community teams.
The team works collaboratively with other professionals, organisations/providers, carers, and families.
As our ideal candidate, you will:
1. Have proven experience of working with individuals with a functional mental health diagnosis.
2. Have knowledge and experience of applying statutory legal frameworks and policies including the Care Act 2014, the Mental Capacity Act 2005, the Mental Health Act 1983 including 117 aftercare, Making Safeguarding Personal, and Continuing Healthcare Frameworks.
3. Root your practice in compassion, social work values, and theories, understanding the why of what we do in our unique profession, to best promote people’s wellbeing.
We currently have a hybrid working policy in place. There is an expectation that candidates can travel into the Borough 3 days per week.
