Duties and Responsibilities:
* Ensures that quality staff are recruited, hired, orientated, and assigned.
* Performs duties necessary to maintain quality clinical standards of care.
* Performs initial screenings of requests for private duty care.
* Develop, implement, and evaluate outcome-oriented client care plans requiring specialized nursing skills. Obtains family and/or client involvement in the development of such a plan whenever possible. Complete all necessary documentation according to private duty division policies and procedures.
* Conduct initial evaluation visits and regularly re-evaluate the nursing, therapy, and personal needs of the clients. Initiate, develop, implement, and make necessary changes to the client's plan of care. Addresses all problems in the plan of care or documents rationale for not doing so.
* Assess the client’s continual care needs and inform the client, the party responsible, the physician, and any other appropriate health professionals as often as needed of any changes in the client's condition.
* Teaches, supervises, and counsels the client and family regarding home nursing procedures and other care needs as appropriate to the client's condition. Ensuring infection control and safety measures are identified, implemented, and documented.
* Maintains accurate, timely, and legible records and reports.
* Works with office manager to ensure appropriate assignments for quality patient and client services.
* Negotiates and/or establishes agreements and contracts for private duty services. Evaluate services provided to ensure the quality of care given to patients and clients.
* Ensures appropriate and adequate inventory of medical supplies used by the private duty staff.
* Performs periodic performance reviews on all employees in the department to ensure that employees are meeting job expectations.
* Performs on-site evaluations of staff to assess the maintenance of safe standards of quality care.
* Addresses patients' clients' and clinicians' complaints or concerns relating to private home care services.
* Ensures knowledge of and competency of the agency’s policies and procedures with staff that affect job duties, responsibilities, and/or employees' conduct through appropriate communication methods to prevent fraud and abuse on behalf of the agency.
* Evaluate the working environment of the assignment to determine whether services should be terminated due to unsafe conditions.
* Maintains the ability to manage a limited caseload to optimize clinical skills as well as provide clinical learning opportunities for both new and existing staff.
* Represents the agency on various committees and associations as it relates to private duty services.
* Assists the private duty team with marketing efforts to promote NVNA and Hospice’s private duty offerings to local hospitals, skilled nursing facilities, assisted living facilities, and local MD offices.
* Performs other agency-related duties as assigned.
Experience/Education:
* Registered Nurse, bachelor’s degree preferred
* 5+ years of nursing experience of which 2 are preferred in a community health setting
* 1+ years of supervisory experience preferred
