Job summary Benfleet Primary Care Network has an opportunity for a care co-ordinator to join our PCN, on a full time basis for 37.5 hours per week, to support our 7 GP practices and assist in re-shaping primary care across our communities. This role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN, and is pivotal in ensuring that all patients receive the best possible care and service. Main duties of the job Support the clinical directors in the delivery of the DES specifications. Answer queries, making, and managing appointments. Contribute to public health campaigns (e.g., flu clinics) through advice or direct care. Provide cross-cover to our other PCN care coordinators at times of annual leave and sickness. Undertake a wide range of administrative duties. Assist the PCN manager with their relationships and engagement with the PCN practices and other stakeholders across the locality. Assist the PCN manager in ensuring the dissemination of information across the member practices. Support in the delivery of training to PCN staff. Support all member practices with data quality issues relating to PCN contracts, enhanced services and IIF/QoF. About us Benfleet PCN was formed in June 2019 with seven like-minded practices wanting to provide a joined up integrated service for our population. The PCN now includes 40 staff members, and we have been known as the accelerator PCN in Mid and South Essex. The PCN provides services for just under 48,000 patients across the patch.
