Theteam is based in Chippenham Community Hospital and provides patient carebetween 07.00 and 22.00 hours. Our core working hours are 08.30- 16.30 withshift patterns including some weekends, early and late shifts although thesecan be flexible. This post is 30 hours over 4 days.
Thisrole is very rewarding and will include supporting a caseload of patients andproviding effective nursing interventions in patients homes and othercommunity settings using evidence-based practice. We also work closely with thewider Multi-Disciplinary team including our therapists and both Primary andSocial Care. Urgent Care Response Advanced Clinical Practitioners are also partof our team.

Qualifications
* Registered Nurse
* To possess a current, valid full driving licence
* Evidence of continuing professional development
* Knowledge of legislation affecting professional practice
* Experience of applying research based information to practice
* Teaching and Assessing in Clinical practice
* Further qualifications relevant to the role
* Non-medical prescribing course
* Awareness of recent Government Papers that may affect future clinical practice
Experience
* Assessment and care planning

* Evidence of innovative and flexible approach to care and the organisation of care
