Full job description
What we ask from you:
1. Working in collaboration with the multi-disciplinary team and colleagues to promote independence and support patient-centred care planning.
2. Provide care directly to patients on the caseload with a range of nursing needs (including palliative care, complex wound care, pelvic health, medication administration).
3. Work autonomously, in line with your scope of practice, competence, and policies.
4. Handover patient information to the wider team and document patient information effectively.
5. Escalate patients' needs as appropriate.
Qualifications and training
Essential: Degree in nursing or equivalent, qualifying September 2024, including NMC registration. Practice supervisor training or a commitment to work towards it.
Desirable: Completed a placement within a community service. Genuine desire to work in a community nursing team.
You must have the ability to travel around the geographical patch.
