Weston Favell Primary Care Centre - Favell Plus GP Surgery
MMWF is a growing PCN (Primary Care Network) which has four GP surgeries. All the surgeries are within Northampton town. We have a diverse population, and having someone who can speak multiple languages will be helpful but not essential.
We are looking for a qualified Dietitian to join our team and help us provide nutritional advice and guidance to our clients. The successful candidate will have a strong knowledge of nutrition and health, as well as an understanding of the principles of diet planning. The Dietitian will be responsible for creating individualised meal plans, providing nutrition education, and monitoring the progress of clients. We are looking for someone who is passionate about helping people achieve their health goals and is willing to go the extra mile to ensure our clients' success.
Main Duties of the Job
An exciting opportunity has arisen for an enthusiastic HCPC registered Dietitian with excellent communication skills and self-motivation.
The role will require the candidate to have a valid UK driver's licence and access to a car. This post requires autonomous working to provide pharmaceutical care to our patients. You will have daily appointments to offer medication reviews and medication advice as well as minor alignments and other contractual obligations including QOF and IIF schemes.
Qualifications
* Qualified, newly or experienced Dietitian
* Must be registered with HCPC
* Must hold a valid UK driver's licence and have access to a car
Meeting nutritional needs in this context can be challenging but very rewarding, with the opportunity to make a significant difference to patient care.
Must be able to offer sound and professional advice in relation to clinical management issues.

This role will build on your existing dietetic skills - clinical knowledge, communication skills, motivational interviewing, caseload management, and collaborative working.
About Us
Primary Care Network working alongside The Mounts Medical Centre, Maple Partnership, Favell Plus Surgery, and Mayfield Surgery.
The successful applicant will join our friendly and supportive PCN. The post will cover a wide range of clinical conditions including management of nutrition support, Diabetes (type 1 and type 2), weight management, dysphagia, food allergies and intolerance, gastrointestinal and eating disorders/disordered eating.
Previous experience of working with a variety of nutritional needs is preferred.
Responsibilities
* Educate patients on nutrition and healthy eating habits.
* Develop individualized meal plans for patients.
* Monitor patient progress and adjust meal plans accordingly.
* Conduct nutritional assessments and provide counseling services.
* Keep up to date with the latest nutrition research and trends.
* Develop recipes and menus that meet nutritional guidelines.
* Work with other healthcare professionals to coordinate care.
Job Responsibilities
To provide a varied clinical caseload across the Network and as a dietitian for our Primary Care Network, you will help to translate the most up-to-date scientific and public health research about nutrition into practical advice to help patients and carers make health-conscious decisions about food and lifestyle. This post will include multidisciplinary working and will deliver high-quality clinical leadership and services, alongside general dietetic work.
Person Specification
Qualifications
* Health & Care Professions Council (HCPC) registration
* Pre-reg MSc in Dietetics under a training programme approved by the British Dietetic Association (BDA)
* Completed or working towards a Supplementary Prescribing qualification
* Evidence of verification against the dietetic primary care roadmap
Knowledge
* Significant experience in applying the dietetic process to include dietetic assessment, interpretation, individual care planning, motivation, monitoring, and evaluation of highly specialized dietetic treatment
* Working knowledge of Microsoft and SystemOne, alongside prescribing data monitoring systems
* Experience of senior-level decision making and delivery in a changing environment
* Working towards advanced clinical practitioner status
* Previous supervisory experience
* Experience of working with a diverse range of stakeholders
* Cognitive behavioural and motivational interviewing approaches/skills
