Our intermediate care services are made up of a range of services including:
Urgent Community Response in addition to our well established Care Call services. We are also one of seven nationally acclaimed accelerator sites for NHS England’s Aging Well programme, delivering a new urgent community response service to people experiencing a health or social care crisis. Our highly skilled team is made up of health and social care professionals and carers working together to support people at home or in the community, contributing to the avoidance of a hospital admission or an admission to respite care.
Intermediate Care at Home Services provide a wide range of care, social work, rehabilitation, therapy, equipment, and assistive technology services supporting people to live well and independently either at home or in the community. We take pride in our GOOD CQC registration for our care services, and our re-ablement outcomes are strong with over 80% of people receiving our service remaining independent at home for three months after leaving our service.
Transfer of Care hub and Intermediate Care Bed Bases support the safe and timely transfer from hospital into settings that best meet individual needs, providing care and rehabilitation to regain independence and undertake assessments for their long-term needs.
