We are looking for a passionate and highly motivated individual who can work as part of a team and as an individual alongside our current Secretarial and Administrative Team to help improve pathways.
To support the Liver Team in proactively tracking patients with liver cirrhosis, diagnostics pathways, and pre/post liver transplant to ensure they are appropriately, and managed progress tracked +/- discussed at the Liver MDT meeting.
To create and monitor a database of liver patients to support their clinical pathway in ensuring they are booked in the appropriate time frame for tests and clinical review appointments, including liver surveillance.
To monitor Liver Patients and ensure that they are seen and followed up in the expected timeframes and to raise with the clinical team if patients are not achieving this.
To use virtual platforms (such as MS Teams) to attend MDT meetings where required.
To ensure accurate data collection to maintain precise patient pathway information on the Liver Tracker for regional data reporting.
