Job Purpose:
The Practice Nurse will work as part of the primary healthcare team providing a range of clinical services and ensuring high-quality patient care. The role involves autonomous clinical practice, chronic disease management, health promotion, minor illness treatment, and participation in care planning.
Main Responsibilities:
1. Clinical Duties
1. Provide assessment, screening, treatment, and care to patients, including management of chronic diseases such as asthma, diabetes, and hypertension.
2. Administer vaccinations and immunizations as per national and practice guidelines.
3. Carry out wound management and dressings, minor illness triage, cervical smears, and sexual health and family planning advice.
4. Conduct health screenings and routine checks, including NHS health checks, lifestyle management, and other assessments.
5. Provide travel health advice and vaccinations.
2. Health Promotion
1. Offer advice on diet, exercise, smoking cessation, and disease prevention.
2. Support patients in developing self-management plans for long-term conditions.
3. Care Management
1. Participate in the care planning process for patients, involving multi-disciplinary team discussions as needed.
2. Conduct regular reviews and update patient records using the practice's electronic system, ensuring accuracy and confidentiality at all times.
3. Work collaboratively with GPs, healthcare assistants, and other practice staff to deliver the best care for patients.
4. Contribute to clinical meetings, audits, and the development of protocols and guidelines for the practice.

Essential Skills and Qualifications:

5. Strong communication and interpersonal skills with a commitment to high standards of care.
