Job Overview: Livewell Southwest Cic is seeking a highly skilled and dedicated Mental Health Nurse Specialist to join our dynamic Community Mental Health Service (CMHS). As a key member of our team, you will play a vital role in providing high-quality care and support to individuals with severe and enduring mental health difficulties.
**Key Responsibilities:**
* Assess the needs of new referrals and develop comprehensive care plans tailored to individual requirements
* Provide specialist nursing care and interventions in various settings, including home and clinic
* Collaborate closely with multidisciplinary professionals, such as Consultant Psychiatrists, Clinical Psychologists, and Social Workers, to ensure seamless service delivery
* Act as a Key Worker for designated patients, coordinating their care and supporting them in achieving their goals
* Participate in clinical audits and quality improvement initiatives to drive excellence in practice
* Stay up-to-date with the latest evidence-based practices, guidelines, and policies, ensuring that your work is informed by best available research and expert knowledge
**Essential Requirements:**
* Registration as a First-Level Registered Nurse in Mental Health with the Nursing and Midwifery Council (NMC)
* Relevant experience working with adults with severe and enduring mental illness, preferably in a community setting
* Strong assessment, communication, leadership, and teamwork skills, with the ability to work autonomously within a team
* Ability to prioritize workload, manage stress, and maintain flexibility and reliability in a fast-paced environment
* Empathy, strong interpersonal skills, and a commitment to delivering person-centered care
**Desirable Qualifications and Experience:**
* Supervisor or assessor training in line with the University of Plymouth or willingness to complete this training
* Life experience in wide-ranging settings and environments
* Knowledge of services and care pathways within Primary, Secondary, and Third Sector agencies, including National Health Service (NHS) Trusts and local authorities
* Mentorship or equivalent supervision training
