We are looking for a Clinical Pharmacist who will work alongside our CoCO Practice Pharmacy Teams and PCN Pharmacist.
This is an exciting role in which you will make a real impact. You will be pivotal in improving the quality of care and operational efficiencies so you will require motivation and passion to deliver an excellent service within CoCO PCN.
Main duties of the job
The post-holder will help patients to get the best from their medicines by communicating medication changes to agreed and approved protocols, improving repeat prescribing processes in General Practice, including promotion of repeat dispensing and online ordering, minimising clinical risk and aiming to reduce wasted medicines. The successful candidate will be supported in terms of learning and professional development through regular meetings with the Practice Team on a variety of topics.

Job responsibilities
The role will include training and development of practice-based prescribing clerks and administrators to support improved co-ordination and effective pharmacy administration within general practice, especially with relation to repeat prescribing systems and processes. You will also be expected to:
1. Develop and facilitate a good working relationship with community pharmacists and other local providers of healthcare.
2. Plan and organise your own workload, including audit and project work, and training sessions for members of the practice team, community pharmacy team, community nurse team, patients, carers, etc.
3. Record and maintain a database of information relating to the work done in the PCN, including any work relating to audits undertaken by you and reference notes relating to clinical/technical information.
4. Maintain registration as a pharmacist and comply with appropriate professional codes.
5. As appropriate to the post, maintain and develop professional competence and expertise, keep up to date with medical/therapeutic evidence and opinion, and local and national service, legislation and policy developments.
6. Attend local, regional and national meetings of relevance.
7. Undertake any other duties commensurate with your grade as agreed with your line manager.
8. Understand that it is the personal responsibility of all employees to comply with all organisational and statutory requirements (e.g. health and safety, equal treatment and diversity, confidentiality and clinical governance).
Person Specification
Qualifications
* Mandatory registration with General Pharmaceutical Council.
* Membership of the Royal Pharmaceutical Society.
* A member of or working towards Faculty membership of the Royal Pharmaceutical Society.
* Independent prescriber.
Experience
* Minimum of 2 years post-qualification experience.
* An appreciation of the nature of GPs and general practices.
* An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
* Excellent interpersonal, influencing and negotiating skills.
* Excellent written and verbal communication skills.
* Able to obtain and analyse complex technical information.
* Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.
* Able to work under pressure and to meet deadlines.
* Produce timely and informative reports.
* Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients).
* In-depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
* Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
* Gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers.
