We are excited to advertise this opportunity for enthusiastic, dynamic nurses to join our team at York on the Frailty Assessment Unit (FAU) and Frailty Same Day Emergency Care (Frailty SDEC). The FAU provides rapid assessment, investigation and treatment for a wide range of medical conditions for patients aged over 80. Our services aim to deliver care to the right person in the right place at the right time and reduce zero length of stay. Successful candidates will have the opportunity to rotate between working on FAU and Frailty SDEC.
A passion and keen interest in elderly care, dementia and delirium is essential.
The role of the successful candidate will be to consistently drive excellent clinical quality standards to this vulnerable patient group and deliver high quality, effective and evidence-based care.
Background knowledge of the safeguarding agenda to protect vulnerable adults is desirable, but not essential, as training will be given. It is our duty to keep patients in our care safe and falls prevention is high on our agenda. Focus will be on patient experience, patient safety and providing assurance that standards are met for every patient.

Main duties of the job
* Maintain a high standard of patient-focused care in accordance with York and Scarborough Teaching Hospitals NHS Trust policies and procedures.
* Maintain a safe and clean working environment.
* Provide assessment, planning, implementation and evaluation of nursing care for patients, ensuring accurate written records are kept.
* Communicate with patients and carers to ensure they have access to appropriate information.
* Promote and maintain positive relationships between all staff involved in the care of the patient.
* Act in accordance with the Nursing and Midwifery Council Code of Conduct.
* Undertake duties on different wards/departments as and when required by service demands.
* Demonstrate personal responsibility for compliance with infection control policies and procedures.
