The Malmesbury and Royal Wootton Bassett Community Team islooking for an inspirational Specialist Community Nurse to lead a team indelivering high standard, quality care to patients within the communitysetting. Post holders will be SPQ qualified or willing to work towards thisqualification. Wiltshire Health and Care is proactive in developing communityspecific training and can offer a supportive and friendly environment in whichto learn new skills.
Our Community Nurses in Wiltshire Health and Care delivercare in patients homes and residential homes as part of a Multi-DisciplinaryTeam, linking closely with the GP Practices and other community services in thelocal areas. We work with the aim of preventing unnecessary admissions tohospital and in assisting people to manage their conditions in their own homes.The Malmesbury Community Team is split into three clusters(Malmesbury/Sherston, Purton/Cricklade and Royal Wootton Bassett) and we are amultidisciplinary team including Nurses, Therapists, Clinical Leads, the HomeFirst Team, Care Coordinators and other staff.
Main duties of the job
The team is based in the Malmesbury Primary Care Centre andoperates a shift pattern from Nursing staff will be expected towork weekends and will be expected to cover early and late shifts., Evidence ofup to date clinical professional development is essential and experience infront line NHS community patient care is preferable.
Applicants need to have excellent clinical skills, welldeveloped communication skills and be able to work under pressure. The role isdemanding but highly rewarding, and we are looking for an enthusiastic,motivated and hardworking individual who can work flexibly and efficiently.
