* Provision of leadership to the Intermediate Care team within the locality
* To be part of the locality's management team, providing leadership to the wider, locality-based Health & Wellbeing Team

* To work as a supportive and inspirational clinical leader within the integrated Locality team, providing a high standard of service-specific expertise
* The leadership role will provide some operational oversight across South Devon Localities
* To work closely with the Locality's GP Practices to ensure that Intermediate Care services are delivered in a safe and effective way; with the outcome of providing an alternative to hospital admission, enabling earlier hospital discharge and preventing early admission to long term care
* To develop and maintain professional working relationships with the Care Homes commissioned to provide Intermediate Care bed-based care

Job responsibilities
Professional Practice
To undertake all aspects of practice as an autonomous practitioner including professional and legal accountability for own work.
To liaise with various multi professionals, clients and carers, in situations which can involve highly complex, sensitive and occasionally contentious information using well developed communication skills.
To be responsible for maintaining own competency to practise through CPD activities, maintaining a portfolio which reflects personal development.
To be responsible for the identification and facilitation of the specific abilities and interests of team members, through performance review and personal development plans.
To be responsible for the maintenance of professional status in line with HCPC/NMC recommendations.
To work with other professional leads and the General Manager to support the locality's wider multi-disciplinary team and to ensure that Professional standards are met across the service.
To actively participate in Safeguarding Adults, Mental Capacity and Best Interests processes, including assessment and participating in meetings as required.
To actively participate in resource allocation processes, attending weekly panel meetings in the locality.
Quality
To be responsible for safe, efficient, and effective delivery of Intermediate Care services, promoting team working in order to develop and consolidate a cohesive and supportive environment.

Leadership
To provide leadership to the Intermediate Care team in the daily MDT meeting, ensuring that targets relating to length of stay and team input are met as well as quality standards and efficient use of resources.
To be responsible for accurate, comprehensive and up-to-date documentation, in line with legal, departmental, governmental, and professional standards of practice.
To be accountable for providing an environment commensurate with life-long learning, and the development of competency frameworks which support the staff development review processes.

Operational
To be responsible for the operational management of the Intermediate Care service within the locality in collaboration with the Physiotherapy Lead.
To provide strong professional leadership, and operational management, for Intermediate Care staff in the locality team on an individual and collective basis, including recruitment and selection, induction, personal development reviews, sickness management, and disciplinary actions.
To ensure that the Locality Intermediate Care service is developing in accordance with the Trust's strategic view of Intermediate Care. To attend Trust IC Leads meetings to maintain consistency of practice.
To chair a regular Locality based Intermediate Care & Community Therapy meeting, alongside the Physiotherapy Lead, to ensure that staff are kept up to date with service developments.
To prioritise and manage the dynamics of an unpredictable working environment through the control and management of effective and efficient use of resources, within budgetary constraints, whilst maintaining high quality, safe delivery of care.
To consider the need for the management of Intermediate Care capacity across the localities, at times of high pressure within the service.
To participate in the implementation of local IT processes to allow for effective data collection, in line with local and national guidelines.
To participate in the development and implementation of both clinical and managerial policies and protocols for the locality.
To follow guidance set by the Trust in relation to decision making about Intermediate Care funded care.
