Ranworth Primary Care Network (PCN) has an opportunity for a care co-ordinator to join our experienced team on a full-time basis, Monday to Friday. This position will be based at the Kennedy Way Medical Centre. The PCN care co-ordinator will help integrate local health, care, council, voluntary services and communities to focus on the needs and priorities of the people who live in our network. The care co-ordinator will support the clinical PCN team and social care professionals to ensure that patients have full access to activities and services resulting in efficient and patient centred health and care planning. The role involves working very closely with the multidisciplinary team (MDT), looking at the patients’ needs and preventing unnecessary hospital admissions. This role is pivotal in ensuring all our patients receive the best possible care and service. The main duties of the role include: Undertake work in line with PCN directed priorities. Proactively identify and work with a cohort of people, who are not living in a residential or care home, to support their personalised care requirements, using the available decision support aids. Ensure regular and consistent communication with the referrer regarding patient progress and any complications or guidance. Raise awareness of health promotion and NHS health checks in practices. Proactively reach out to patients that do not attend the surgery. Support national screening programmes. Support immunization programmes. Monitor referrals to ensure tasks are completed and care delivered by keeping in regular telephone contact Direct liaison with multi agencies to coordinate care for patients. Refer to PCN social prescribing link workers and Tendring Community Voluntary Services, where a patient is identified as potentially benefitting from this service. Support patient/carer contact, and collate patient and carer feedback on their experiences. Act as the first port of call for patients, in their caseload in relation to their care. Help people to manage their needs, answering their queries and supporting them to make appointments. Support people to take up training, employment and access appropriate benefits where eligible. About the Candidate If you are a skilled care co-ordinator with excellent person-centred planning experience, with local knowledge of key support and advice and support to our patient population, then we'd love to hear from you. You will need to have the following skills/qualifications : Ability to organise, plan and prioritise on own initiative, including when under pressure and meeting deadlines. Experience of supporting people, their families and carers in a related role (including unpaid work). You will have experience of working within healthcare or the voluntary/community sector, supporting vulnerable groups. Experience of partnership/collaborative working of building relationships across a variety of organisations. Experience of working with healthcare professionals (including unpaid work) Diploma Level 2 in Health and Social Care or equivalent qualification or experience. You will be familiar on local resources and services and how to access them. You will be motivated by helping people, with care and empathy. Experience of completing holistic person centred care planning assessments.
