Job summary Come and join our team The role of the Care Co-ordinator is pivotal within primary care as the interface between service users, carers, primary care, secondary care, community care, social care, mental health, out of hours and voluntary organisations. The Care Co-ordinator will be part of the ARCH Integrated Community team and working alongside GP Practices within Amber Valley. The Care Co-ordinators provide an administrative, non clinical role, and applications are welcome from anyone with Health or social care experience. The Care Co-ordinator will contribute to tackling inequalities in health and social care particularly regarding individuals with long-term conditions.
