KEY RESULT AREAS To work primarily independently and on own initiative with minimal supervision to carry out the duties of the post. To track patients referred as suspected cancer to ensure all diagnostics and follow-up appointments are booked to meet agreed timescales/targets working in line with Next Steps. Track patient test results to determine what further action is necessary, liaising with the clinician and/or ensuring that relevant information is available for discussion at the MDT. Provide administrative support and facilitate MDT meetings including preparing, organising and circulating agendas ensuring all members of the MDT are advised of the meeting date/time/location. Responsible for recording and collating attendance of MDT meetings in accordance with Peer review requirements. Utilise knowledge to determine which patients notes and images will be needed for discussion. Locate, collect and return the notes for the MDT as required. Record the outcome of the MDT for each patient and minute the MDT meetings accurately and circulate as appropriate. Ensuring communication of outcomes with other Healthcare professionals as requires e.g. GPs. Take responsibility following the MDTs for ensuring that any outcomes identified as the responsibility of the Cancer Pathway Navigator are actioned in a timely manner, liaising with staff in various departments as necessary. To be responsible for ensuring that the patient does not experience unnecessary delay on the cancer pathway. Monitor the patients current status and communicate as required with the appropriate consultant to ensure that diagnostic/staging tests and treatment are arranged in a timely manner, and to chase results as required. Work collaboratively with medical secretaries, clinic coordinators, nurse specialists and other staff within the Trust to ensure appropriate management of patient information and coordination of patient pathway. Extract clinical and waiting times information from the patients notes to record accurately on the MDT data sheet, the MDT minutes and the Cancer Waiting times Database. Using knowledge of cancer waiting times, ensure the timely, accurate insertion of patient information onto the Cancer Information System database. To negotiate appointments for patients with other members/departments of the Trust so ensuring the targets are met. To complete data reports as required in relation to Cancer Outcomes Services Dataset, and other reports as required by the Tumour site, Cancer Centre or Trust. Inform Cancer Centre Manager and MDT Chairs or clinicians responsible for the Patient of patients who are going to breach the cancer waiting times targets. Take responsibility for the prioritisation of workload. Develop systems to improve effectiveness of the post. Be familiar with tumour site specific pathway information, including databases/spread sheets or National Databases/Registries and enter data in a timely manner from the completed data collection proformas. Arrange room and facilities bookings for MDT meetings. Implementation of the Operational Policy for relevant MDTs. The post holder will also be expected to participate in meetings for the ongoing development of cancer policies. Co-operation with Cancer Services MDT Team Leaders, Assistant Cancer Managers, Cancer Lead Managers and Peer Review Lead, in all aspects of information gathering for Peer Review. Responsibility for the effective facilitation of video conferenced meetings, including the setting up and operation of equipment and the room. Attend and contribute to the AGMs and business Meetings of the MDTs. Provide cover for patient tracking and MDT team meetings and other duties as appropriate.
