Within the Community Services Division of Wrightington Wigan and Leigh NHS Foundation Trust, The Frailty SDEC supports individuals who require both short term medical interventions and a period of bed-based, Nurse & Therapy led rehabilitation. It will support individuals to achieve specific goals which enable safe discharge back into the community or their own home.
Frailty SDEC is operational 24/7/365 so shifts will be day or night. This is a full time role (37.5 hours) on a 12 month fixed term basis to cover maternity leave. If the substantive postholder does not return, it may be made permanent.
Please note NMC registration is a requirement of this role.
Main duties of the job

* To communicate effectively with all members of multi-disciplinary team regarding patient care.
* To assist with the investigation and timely resolution of complaints and PALS enquiries.
