The role will be across Kirklees, utilizing our community bases at Beckside Court in Batley and Folly Hall in Huddersfield, working Monday to Friday 9am-5pm. Flexible working options are available.
You will need to be a car driver and have the use of a vehicle to undertake any necessary travel in connection with the duties.

Main duties of the job
A key element of the role will be care coordination, supporting colleagues, duty worker, and prioritization of workload in times of high clinical demand.
Undertake assessments and review care plans of service users which promote engagement, stability, recovery, and risk management.
Manage a varied caseload covering functional and organic mental health presentations.
Work autonomously but contribute to the functioning of a multi-disciplinary team.
Work with service users with both short-term interventions and severe and enduring mental health needs to promote health and well-being.
Document care delivery to a high standard in line with professional standards, local, and national guidance.
Recognize situations that may be detrimental to the health and well-being of the service user and initiate action that may be required.
Support student education when allocated to the team and contribute to the preceptorship and education of newly qualified staff where appropriate.
Administration of depot medication in the community.
Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.

Job responsibilities
Has continuing responsibility for the provision of skilled interventions to people with mental health problems in the community. Works within a multi-disciplinary team as a member of a Core CMHT.
Management of own caseload in line with C.P.A. procedures and risk management protocols.
Provision of leadership to the Core Team in respect of psycho-social interventions to people and carers.
Supervision and support of other team members.
Participation in service development and clinical governance structures.
Assesses needs and identifies problems relevant to the care of people referred.
Devises a plan of care in partnership with the person and carers. Implements the planned programme of care to ensure a high standard is achieved.
Reviews the effectiveness of the care provided and where appropriate initiates any action.
Advises and supports the person and carers to promote health and well-being and to prevent illness.
Recognizes situations that may be detrimental to the health and well-being of the person and initiates action that may be required.
Establishes and maintains satisfactory communication with all agencies involved in the person's care in both hospital and community.
Functions as a member of a multi-disciplinary team.
Management of enquiries and direct referrals of people in accordance with the Core C.M.H.T. Operational Policy and participates in the team duty rota.
Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.
Responsible for acting up in the absence of the team leader.
Maintains accurate patient care records in accordance with professional codes of conduct and trust procedures and ensures records are made available when required by the appropriate agency.
Participates in the formulation of policies at unit level.
Maintains accurate mileage records.
Manages caseload with due economy of cost and time.
Ensures safe carriage, storage, and administration of drugs prescribed (registered nurses only).
Responsible for the ordering of stock with due economy.
Produces monthly statistics in line with trust directives.

Training

* Proven ability in assessing the needs and strengths of people and their carers.
* Ability to deliver a range of therapeutic techniques which can be incorporated into one-to-one and/or group interventions.
* Working within an integrated multi-disciplinary environment and co-working with other teams within team meetings, case conferences, and professional meetings.
* Ability to support colleagues and supervise students/trainees.
* Experience of working holistically with adults with an emphasis on coexisting mental and physical health needs that impact on well-being.

Qualifications
* Registered Mental Health Nurse / Social Worker / Occupational therapist.
* Experience of mentoring/supporting learners or ability to demonstrate the skills required for effective mentoring.
* Expected to work towards Practice Educator.
Special Knowledge & Skills
* Working knowledge of recent government legislation e.g. Mental Health Act, Health & Safety, Care Programme Approach, Care Act, and Safeguarding.
* Able to work incorporating principles of Clinical Governance.
* Knowledge of working collaboratively with people and their carers to ensure their involvement in the assessment and care planning process.
* Shows awareness of cultural diversity and is able to incorporate this into care planning and the delivery of care.
* Knowledge and evidence of using IT systems.
* Good written and verbal communication skills.
* Knowledge of audit processes.
* Knowledge of carer services/issues/assessment.
Physical Attributes
