Although the successful candidate may be required to work in any Lowestoft Practice. This position will mainly be based in Alexandra Road and Crestview Surgery. Coach and motivate through multiply sessions using personalised goal setting to implement a patients personalised health and care plan for long term lifestyle and behaviour change. Identifying frequent attenders and work with patients with lower activation scores to understand and increase their level of knowledge, skills and confidence to self-management their health conditions. Pain management psychological element Mental health (well managed) Working with carers Referring to Social Prescribing Link Workers and the broader PCN MDT to enable patients to take control of their health and wellbeing. Supporting return to work. Supporting participation in community activities/exercise to improve mental and physical wellbeing. Addressing loneliness Supporting recovery from drug and alcohol addiction. Lifestyle group education sessions / webinars Virtual / group consultations To be responsible for day-to-day planning and personal caseloads To develop yourself and the roll through training as well as coaching and clinical supervision. Identify when patient needs are beyond the scope of a HWbC and refer to the appropriate health professional or organisation. Proactively work with referral partners to maximise appropriate referrals and to promote the service.
