PART TIME 20 HOURS PER WEEK
We are looking to recruit a health care assistant to support the Living Well Programme at Slough Older People’s Mental Health Service.
Are you committed to enabling older people with mental health challenges to live meaningful and fulfilling lives, following a diagnosis?
The successful applicant will work within the multi-disciplinary team and support the treatment of people with mental health needs in their own homes and engage in groups and activities as part of the service offer. The successful applicant will also assist in implementing the individualised care treatment plan by engaging with the individual as required.
Main duties of the job
The main duties of the role include:
* To assist and support the assessment and treatment of patients within the service
* To accompany staff on home visits and assessments when required
* To support people in their own homes by focusing on wellbeing and optimising independence
* Transport patients and carers as required
* To support group activities as part of the Living Well Program.
