The role of the Dermatology GPwER Offer dermatology consultations for assessment of patients referred into CDS services clinics with use of dermoscopy as appropriate Offer procedures such as cryotherapy, injections and surgical intervention as appropriate Arrange investigation, offer management plans for GP implementation (returning patients to GP care) or to offer follow up (either face to face or as a telephone consultation) or onward referral to secondary care as needed Maintain adequate and timely records of consultations, investigations, prescribing and procedures using the clinical system (currently EMIS) and its templates &/or a separate database where appropriate (eg biopsies) Ensure timely review of results with communication of the outcome to the patient or their carer and/or referring clinician as appropriate. Complete computerised outcome letters to the referring clinician/ patient/ carer and liaise with the administration team to ensure that letters reach them Prescribe in line with local dermatology formularies (Southwark/ South East London APC/ JMF) Practise in a way that seeks to comply with the SE London Treatment Access Policy. Practise within national 2WW/ NICE/ IOG/ BAD/PCDS/ local dermatology guidelines Practise within their competence/ acknowledge limitations and decline duties for which theyre not qualified. Liaise/ seek advice from the community dermatology consultant/ GPwER colleagues for optimal delivery of patient care and to address their training needs. It is anticipated that the GPwERs will work alongside or meet/ meet with a clinical supervisor face to face/ virtual clinic to discuss patients at least monthly Complete relevant activity documentation (by hand or within computer templates as requested) to allow tracking of KPI Complete mandatory training and comply with relevant regulations Handle data/ images/ patient information in a GDPR compliant manner eg ensuring use of nhs.net email addresses for transfer of patient specific information Take reasonable care for their safety and that of others in both actions and omissions. Report and seek immediate advice in the event of a needle stick injury or other injury sustained whilst delivering care Report significant or near miss events to clinical supervisor and manager (GSTT for clinics in North of CCG, IHL for clinics in South) at the earliest opportunity Triage Triage incoming service referrals or requests for advice and guidance and offer cover for colleagues in this way during their annual leave/ sickness. This will be renumerated separately unless it has been agreed that it forms part of a regular session. Meetings Attend or participate in a regular (initially weekly) brief administrative and clinical meeting with colleagues. Support the delivery of dermatology education to referring clinicians who may sit in on clinics, attend educational sessions offered by the service or attend an annual PLT. Support the development of dermatology guidelines Clinical governance and audit Be covered by GSTT indemnity for all activities carried out on behalf of the community dermatology service. Comply with IHLs and GSTTs policies and procedures Complete occupational health screening, maintain appropriate immunisation e.g. blood borne diseases, tetanus, polio and practise in a safe manner Maintain their GMC registration and be on a CCG medical practitioners list offering at least 2 GMS session per week; to provide evidence of this and to comply with appropriate standards of conduct Participate in audits of both service provision and clinical practise (surgical audits annually) Complete and document their training, CPD and assessments with a clinical supervisor(s) (usually from within the service) and maintain a reflective patient diary as necessary to achieve RCGP GPwER Dermatology accreditation at the earliest reasonable opportunity (The fee for one GPwER accreditation submission will be paid by the Southwark Community Dermatology service). Complete CPD to maintain competent practice as a GPwER dermatology in line with RCGP 2018 Guidance and competences to support the accreditation of GPs with Extended Roles (GPwERs); this may, at different times, be achieved by working alongside a consultant colleague, bringing patients to consultant led clinics in the community or secondary care, in virtual clinics, by attending the monthly St Johns academy dermatology meeting or Kings complex cases meeting (both Thursday pm). Fees for courses will usually be the responsibility of the GPwER (St Johns Academy courses may be available at a reduced rate). Where a GPwER attends a supervision clinic this will be in lieu of another clinic, likewise where MDT attendance is necessary for service delivery (eg level 3 GPWERs x 4-5 / annum where managing BCCs) this will be renumerated. Other CPD attendance will otherwise usually be outside of clinic commitments but be included within the GPwERs contractual. Complete an annual dermatology appraisal (this will usually be with a clinician from the community dermatology service) in addition to a standard GP appraisal and revalidation cycle GPwERs will participate in research programmes if appropriate Patient and Public involvement The GPwER will endeavour to ensure that services are responsive to the needs of people affected by skin conditions and will assist in patient surveys as requested.
