The Office of Healthcare Innovation and Quality in Commonwealth Medicine provides consultation and technical assistance to payer and provider organizations in redesigning the healthcare delivery system to achieve the Triple Aim. Our experts support the design and implementation of new models of care delivery, including the Patient Centered Medical Home, Accountable Care Organizations, and the integration of behavioral and physical health care.
Under the direction of the Deputy Chief Medical Officer of Commonwealth Medicine (CWM)/Senior Director of the Office of Healthcare Innovation & Quality (HI-Q), this position serves as the lead facilitator and catalyst for improving the clinical and operational performance of clinical practices and provider organizations implementing the Patient Centered Medical Home (PCMH) model of care.
The PCMH Quality Improvement Director and Transformation Expert provides leadership in planning, reporting, evaluating, and facilitating the achievement of practice transformation and performance improvement. He/she acts as a liaison between participating practice sites and all stakeholders associated with CWM’s PCMH activities. This person will manage and supervise a team of Transformation Specialists overseeing day-to-day PCMH practice facilitation activities, and will lead the design and implementation of learning collaboratives for practices participating in PCMH and other healthcare delivery reform initiatives. Additionally, the PCMH Quality Improvement Director and Transformation Expert will provide expert advice to PCMH project leaders on the development, communication and implementation of a clear vision and strategy as to how practices will successfully transform into medical homes.
* Provide consultative supervision to Transformation Specialists or Practice Facilitators from outside of UMMS per contractual or other arrangements
* Coordinate efforts of Transformation Specialists with PCMH learning collaborative efforts and the efforts of other PCMH initiative stakeholders from within and outside UMMS
* Lead the design and implementation of learning collaboratives for practices participating in PCMH and other health care delivery reform initiatives. Learning collaborative activities may include webinars, videos, panel discussions, in-person learning sessions, conference calls or office hours, posting of practice resources on a learning collaborative website
* Lead learning collaborative faculty, transformation specialists and contracted learning collaborative consultants to develop and refine learning collaborative curricula and presentations using the learning modalities described above and serves as a presenter in learning collaborative activities
* Oversee the collection and analysis of data evaluating customer satisfaction and impact of the services of the Transformation Specialists and of the learning collaborative and utilizes this information to improve these transformation services.
* Oversee the collection, integration and evaluation of diverse data from participating practices related to clinical and operational performance and medical home transformation activities; identifies trends and issues; and uses this information to inform learning collaborative curricula and the work of the Transformation Specialists
* Communicate practice performance improvement and medical home transformation activities, results and issues by developing reports with verbal, written and visual presentations to leadership teams, committees, and other key stakeholders. Work with internal UMMS/Commonwealth Medicine administration to develop necessary business plans and financial analyses for medical home and practice redesign initiatives
* Develop and oversee budgets for PCMH transformation services Attend variety of meetings, conferences, seminars as required or directed
A Master’s Degree in Nursing, Public Health, Business Administration, Education, or a related field with a minimum of 8-10 years of related professional experience in quality/process improvement and organizational change management with at least five of these years in progressively responsible management roles, and experience in a clinical setting is required. Prior national experience in practice transformation to the PCMH model, including PCMH demonstrations, PCMH learning collaboratives, practice facilitation and measurement of PCMH components is preferred with proven administrative and management ability in complex environments. Additionally, demonstrated performance in program development and implementation, demonstrated ability to analyze and utilize data, excellent oral and written communication skills, and innovation, flexibility and creativity is required.
