An exciting opportunity has arisen to join the Children and Young People’s Continuing Care / Complex Care Team. Working in partnership with children, young people and their families, our innovative and friendly Complex Health Team plan and commission the delivery of complex and continuing care to our population of under 18s. This pivotal role offers a chance to be part of the wider Children, Young People and Maternity team, who work in partnership with acute providers, primary care, Norfolk and Suffolk County Councils, commissioned health and social care services, the third sector, and communities. We believe in an integrated approach and work as a system to meet the needs of Norfolk and Waveney’s children and young people in order to achieve the best possible outcomes.
We are looking for an experienced Registered Children’s Nurse / Learning Disability Nurse to work within the Children and Young People’s clinical team. Your role will facilitate a holistic assessment process to identify healthcare needs, to plan, source, coordinate, and oversee appropriate provision to meet those needs and set outcomes to help children and young people flourish. Provision will be reviewed on a regular basis to ensure that it best meets the needs of the young person as they develop. You will be pivotal in meeting the needs of children and young people with additional needs, such as autism and learning disabilities, coordinating support and oversight through Care Education clinical reviews.
You will work closely with partners throughout Norfolk and Waveney, including Social Services, the Special Educational Needs Service, Schools, and local health providers to support children and young people with additional needs to achieve their potential; this is an opportunity to develop an understanding of provision beyond healthcare settings and to make a lasting positive difference to the individuals we support.
You should have experience of supporting children across education and social care with SEND. Experience in Continuing Care assessments is preferred but not essential.
To work in partnership with CYP and their families, the post holder leads the implementation of the Children’s Continuing Care Framework to include personal health budgets and fast track in line with national policy, guidance, and frameworks to ensure the ICB is fully compliant with legal and statutory requirements.
To ensure that care packages and placement proposals are clinically appropriate, person-centred, have all the agreed documentation in place, and are making best use of agreed resources to meet the agreed outcomes.
To work closely with partner agencies and the local authorities to continue to support the implementation and development of jointly resourced packages of care, ensuring that the child's needs are met safely when these have been agreed and the responsibility of health.
To take a lead role in coordinating the management of complex cases across the health community, ensuring at all times that patient choice, safety, and quality remain uppermost in informing decisions. Ensure appropriate engagement with patients, family members, and key stakeholders.
To advise and support children, young people, their families, and carers throughout the process of assessment and case management of support packages.
To complete personalised health assessments for children and young people and chair multi-agency meetings to inform the decision-making process within the Continuing Care Framework.
To attend and contribute to the Children’s Continuing Care eligibility panel.
To participate in Care and Treatment Reviews (CTRs) for CYP in tier 4 provision.
To monitor contracts with continuing care and complex health care providers ensuring monthly reports are received and reviewed.
To undertake an annual quality visit of all children and young people who have a continuing care package as a minimum.
To ensure the delivery of a high-quality, responsive service to meet the health needs of the identified population.
To prepare for the transition of young people with care packages moving from children to adult services.
Ensure accurate, open communication and coordination with a range of organisations and individuals.
Research and draft correspondence and papers and ensure the management of specific tasks, lead reporting and analysis across a range of specialties, functions, and projects.
Be a key member of the team as well as supporting effective communication and stakeholder management, both internally and externally.
To provide contemporaneous records and reports adhering to NMC standards for record keeping.
To develop commissioning knowledge with the support of the children and young people’s commissioning team.
Act in an educational and advisory role to primary care staff and other partner organisations to promote and share knowledge concerning complex health care.
To attend national events in relation to the role and represent the ICB in the evolving agenda in relation to all elements of the role.
