An exciting opportunity has arisen for an ANP/AP or trainee ANP/AP to join the Advanced practice team within Perth & Kinross Health & Social Care Partnership as part of the Prison Healthcare Team. This post will be based in the specialty of Substance Use.
We are seeking an individual who is flexible, hardworking and professional.
The post holder will utilise their advanced clinical skills and knowledge incorporating a high level of critical thinking and clinical judgement skills. This will involve working autonomously with high level of decision making and problem solving. They will have the ability to make professional autonomous clinical decisions through history taking, physical examination and problem solving for adult patients who are likely to have complex underlying co-morbidities/healthcare requirements.
The post holder will communicate at all levels across boundaries and work closely with wider clinical team members. The post holder will also work collaboratively with the multidisciplinary team to meet the needs of patients, supporting the delivery of policy and procedures, and providing Clinical leadership as required.
The post holder will work closely with people to identify clinical issues quickly and aim to support avoidable deterioration, admission/readmission to secondary care supporting person-centred Future care planning. You will manage referrals and liaise, closely with consultant connect/OOH/SAS/SPS to ensure smooth, efficient and person centred transitions of care.
The post holder will play a key role in the leadership and development of advanced nursing practice within Prison Healthcare and will participate in the delivery of specialist education and training programmes and work collaboratively with health and social care colleagues across the partnership to deliver high quality evidence based care.
The post holder will also play a key role in enhancing patient experience, assuring clinical governance and supporting the Senior Leadership Team in the improvement and continued development of the service.
The post holder will work across the Prison Healthcare Team in the 3 Scottish Prison Service establishments in Tayside (HMP Perth, HMP Caste Huntly and HMP and YOI Bella Centre).

The successful candidate should preferably be a suitably qualified ANP/AP with experience in a prison/secure environment OR an experienced Registered practitioner who possesses the commitment and willingness to undertake the ANP training pathway (Annexe 21) is essential. Annex 21 paragraph 2 (i) of the Agenda for change terms and conditions will apply until successful completion of the ANP training programme.
You must have the personal drive, clinical knowledge and commitment to undertake this challenging and rewarding role. The ability to travel across Tayside is essential. Please see Job description for other requirements of this post.
