The Children’s Rapid Response Service has an exciting opportunity as it continues to expand the team. The service works collaboratively with the Community Children’s Nursing & Palliative Care Team, with the primary aim to prevent and reduce hospital attendances, admissions, and length of stay for children already known to the Community Children’s Nursing & Palliative Team.
The emphasis of this service is to improve the quality of life for children and families with complex healthcare needs, offering them greater choice around their chosen place of care.
Are you an enthusiastic, dynamic, flexible, and motivated experienced Nurse or Nurse Practitioner, with a strong knowledge of Children’s Nursing? Clinical expertise and good communication skills are required, as well as excellent time management and prioritisation skills which are essential for this role. The service is open from 07:30 – 22:00 across 7 days and there is flexibility to work short shifts or long days.
Main duties of the job
We are looking for experienced children’s nurses that have extensive experience in assessment with children with complex healthcare needs.
You must have the ability to prioritise your workload using advanced assessment skills with the ability to provide a symptom management plan of care to children/young people during times of acute deterioration or illness.
The role requires undertaking clinical assessments of a sick child or young person and emergency enteral feeding management to prevent hospital admissions.
Detailed job description and main responsibilities
1. Work in accordance with NMC Code of Professional Practice and Scope of Professional Practice.
2. Professionally and legally accountable and responsible for all activities.
3. Provide comprehensive evidenced based and skilled nursing care to children within their own home and/or other community settings.
4. Ensure assessment, planning and delivery of holistic care is of the highest standard utilising an agreed nursing model.
5. Work in partnership with children, families and carers to provide optimum individual outcomes acknowledging children and family’s personal preferences and lifestyles.
6. Liaise and work in collaboration with acute services, primary healthcare team, and other agencies when planning and implementing programmes of care.
7. Provide the clinical lead in care packages funded by the Community Complex Care team when allocated to the Community Children’s Nursing (CCN) caseload.
8. Provide health education information to the child/young person, family, and carers to promote a healthy lifestyle through health promotion activities as required.
9. Plan and participate in teaching packages to families and carers to ensure evidenced based care is delivered.
10. Advise the Team Leader, Clinical Lead, or Service Manager of any circumstances which give cause for concern in relation to patient care and/or any concerns raised by the primary healthcare team or partner agencies.
11. Undertake home visits to children registered with GP’s within Birmingham as required.
12. Undertake home visit risk assessments to ensure safety during visits especially those during unsociable hours.
13. Obtain informed consent from Children, Parents, and Carers prior to undertaking care.
14. Maintain clear, concise, and contemporaneous records adhering to BCHC record keeping policies.
15. Provide a flexible service to accommodate the working environment.
Person specification
Qualifications / Training
Essential criteria: RSCN/RN Child Branch, ENB 998 (Teaching & Assessing in Clinical Practice) or equivalent, Community Children’s Nursing qualification or equivalent.
Desirable criteria: Children’s Palliative Care experience, Independent supplementary prescriber or equivalent.
Experience
Essential criteria: Significant experience of working as a Children’s Nurse either RSCN/RN Child.
Desirable criteria: Experience of working with children with life limiting/threatening illnesses and end of life.
Skills/Knowledge
Essential criteria:
1. Ability to manage a team on a day-to-day basis in the absence of the Team Leader.
2. Demonstrate knowledge relating to child protection issues and procedures.
3. Evidence of assessment skills and use of nursing process and nursing models.
4. Demonstrate sound use and application of NMC Code of Conduct and scope of professional care.
5. Demonstrate knowledge of current legislative changes affecting NHS and Social Services particularly those affecting community care.
6. Demonstrate knowledge and application of current clinical trends and practices inclusive of evidence-based practice in relation to Children’s Community Nursing.
7. Ability to teach relatives, patients, clients, and staff across all areas of self-care and appropriate nursing practice.
8. Demonstrate knowledge of standard setting and audit processes.
9. Demonstrate individual responsibility in respect to health and safety.
10. Ability to demonstrate written skills that are concise, accurate, relevant, and legible.
