An exciting opportunity has arisen for a Cancer Care Co-ordinator, who is highly motivated with excellent communication skills, to join the Head & Neck (H&N) Team. The H&N team comprises of Dietitians, Speech & Language Therapists (SLT) and Clinical Nurses Specialists (CNS).
The postholder will enable and assist the H&N team in delivering high standards of care to all patients with a Head & Neck cancer diagnosis. They will provide support and information to patients and families throughout their cancer pathway.
The post holder will be working with pre and post treatment oncology and surgical patients, including laryngectomy. The post requires excellent verbal and written communication skills, including an ability to deal with distressing/sensitive information in a calm and confident manner. The ability to organise and work to deadlines, ensuring quality standards is essential as well as the ability to work on your own initiative and as part of a team.
Main duties of the job
The H&N Cancer Care Co-Ordinator will work with the H&N team to ensure the delivery of high quality, patient centred care within the Mid Yorkshire Teaching Trust for patients who have had a diagnosis of Head & Neck Cancer. The role includes but is not limited to:
* Triage incoming telephone calls.
* Act as point of contact to patients, GP practices and other health professionals.
* Assist the H&N team with MDT actions as appropriate.
* Assist where needed in Nurse/AHP led clinics and inpatient setting.
* To monitor and replenish stock of patient related equipment.
* To be able to complete a holistic assessment and manage immediate concerns, with support from the H&N team.
* Book patients into CNS and AHP clinics.

Job responsibilities
* Triage incoming telephone calls, managing onward referrals and signposting to appropriate person or service.
* To act as point of contact to patients, GP practices and other health professionals.
* Assist the H&N team with MDT actions as appropriate.
* Input information onto relevant computer systems to ensure effective communication between all individuals involved in the patient pathway.
* Develop and use appropriate communication skills to support contact with patients face to face in clinics, wards and on the telephone.
* Undertake routine phone calls to patients following surgery, radiotherapy and chemotherapy, and involve the AHP/CNS as needed.
* Assist where needed in Nurse/AHP led clinics and inpatient setting.
* To monitor and replenish stock of all patient information leaflets and order as required.
* To monitor and replenish stock of patient related equipment.
* Support AHP/CNS team in collection of data to support audits and patient surveys.
* Understand the patient treatment and information pathways and assist patients in accessing treatment and resources.
* To be able to complete a holistic assessment and manage immediate concerns, with support from the H&N team.
* To attend local MDT meetings when required.
* To be able to communicate with the patients' friends and relatives and involve them in care as appropriate.
* To be aware of personal limitations and to seek assistance from other team members as needed.
* To monitor availability of investigation results and once available to ensure they are made available to the relevant team member.
* To ensure clear documentation of all interventions made with patient, relative or staff member.
