This is an exciting opportunity to play a key role in the development of Population Health Management (PHM) capability in Hampshire and Isle of Wight. This is a pivotal role to the development and implementation of a high impact and high-profile programme in one of the most exciting new areas of health and wellbeing.
Population health management is a key contributor to ensuring patients receive the care they need, in the right place and at the right time which is the top priority for the Hampshire and Isle of Wight Integrated Care System (ICS).
The service maintains, monitors and develops analytics within the population health platform, supplying data and insight into decision makers, programmes, projects and priorities.
This is a hybrid role. The team primarily works at home, but you will need to be able to attend face-to-face meetings within an office on at least a weekly basis.

You will be responsible for overseeing the day-to-day implementation and future roadmap of the population health analytics platform, successfully bringing in data from multiple organisations across health and care, and supporting the planning and development of analytics.
As Senior Delivery Manager, you will work with the partnership (including suppliers) to design, develop and manage the implementation of the capability across the infrastructure and intelligence workstreams. Using your ability to be intellectually flexible and to look beyond existing structures and ways of working, you will autonomously manage and progress the delivery of the projects.
The post is responsible for providing professional expertise including business and analytical processes for the programme. As a result, the post holder will identify current processes, support discussions and advise colleagues as to how best practice might be adopted in the definition of future processes, document those processes as they are agreed and oversee their delivery.
You will report to the Associate Director of Population Health Analytics. You may be asked to line manage a small number of team members to ensure successful delivery.
Operational Responsibilities:
1. Support the design, development and ongoing maintenance of implementation plans and associated work packages in collaboration with our partners and suppliers.
2. Manage programme resources to develop, test and deploy intelligence/intervention products and outputs.
3. Management of risks and issues associated with the implementation and deployment of the capability.
4. Define engagement and training strategies to ensure the solutions deliver the desired impact and outcomes.
5. Engage with end-users, gather feedback and co-produce training materials.
6. Support demonstrating value for money through embedding benefits realisation of agreed benefits.
7. Design and develop Business as Usual (BAU) processes and procedures to enable a sustainable PHM infrastructure solution for the ICS.
Financial and Physical Resources:
1. Responsible for matching the available budget for each workstream to the appropriate staff resources and projects.
2. Prepare financial reports to Directors and the Board as requested and directed.
3. Act in a way that is compliant with Standing Orders and Standing Financial Instructions in the discharge of this responsibility.
Staff Management:
1. Responsible for the day-to-day management of staff associated with implementation of the capability.
2. Provide advice and guidance to Project Support Officers to support and develop them in their roles.
Information Management:
1. Develop the acquisition, organisation, provision and use of knowledge and information.
2. Collaboratively draft and present reports summarising status on issues, appraising outcomes, and providing progress reports for a range of stakeholder audiences.
