Our “Strengths-Based Model of Practice” supports a multidisciplinary approach to enabling and supporting people who live with issues caused by disability, frailty, and ageing.
We are looking to appoint a part time Social Worker who will be based within the Early Intervention and Prevention Hub. This includes the short-term social work, community support workers and the front door response in Adult Social Care. The team supports a multi-disciplinary approach, with a focus on adults who have multi-faceted complex needs and cognitive decline. This service focuses on outcomes for individuals, maximising individual potential and minimising risk to independence with an emphasis on choice and control.
A recognised Social Work Qualification and registration with the Care Council for Wales is essential.
The ability to greet customers through the medium of Welsh is a requirement for this post.
Protecting children, young people or adults at risk is a core responsibility of all council employees.
An Enhanced with Adults Barred list criminal records check by the Disclosure & Barring Service (DBS) is a requirement for this post.
A valid driving licence is requirement for this post.
