We are seeking Health Care Assistants to work in the community on a temporary, as-needed basis, to support our team, department, and organization in achieving the Trust's mission of "Best care by the best people."
You will assist the clinical team in delivering patient nursing and rehabilitation care by working closely with qualified nursing and therapy staff. As part of the team, you will help implement care and rehabilitation programs to support patients in regaining confidence and maintaining activities of daily living.
Candidates must meet the person specifications and hold the necessary qualifications for the role.
The successful applicant must be flexible, a strong team player, able to work under pressure, possess excellent interpersonal skills, and communicate effectively with patients, carers, and other members of the multidisciplinary team (MDT).
Applicants must hold a valid UK driving license and have access to a car for work purposes. Reasonable adjustments will be made to accommodate the needs of disabled applicants.
Please note that an NHS Care Certificate is required to be considered for this position.

Key Responsibilities:
To be responsible for organising and prioritising own delegated workload.
To have organisational knowledge relating to Trust protocols and procedures and adhere to them, particularly administration of medicine and moving and handling.
To be responsible for providing accurate records of information required by the Trust for audit purposes.
To ensure effective risk management at team level by accident/incident reporting, assessing and controlling risk and ensuring residual risks are added to the Trust register.
To plan and organise own delegated work.
Togive holistic care to patients in the community, such as in the patients home, healthcentre / clinic and residential home.
