We have an exciting opportunity for an experienced nurse to join the Rheumatology department at Neath Port Talbot Hospital.
The band 5 Rheumatology nurse will support the existing Clinical Nurse Specialists to support patients in the community.
The applicant for this role will need to be flexible and will support the Osteoporosis Nurse Specialist, Biological Nurse Specialist, and DMARD nurse.
The applicant will need to be digitally competent as the role involves using several computer systems - full training on the systems will be given.
The role is predominantly office-based and there is limited face-to-face contact with patients. Thus excellent communication skills are needed for this role, and the ability to communicate over the telephone is essential.
The role is varied and covers several specialties within rheumatology; thus the applicant will need good organisational skills.
Main duties of the job
1. Monitoring of bloods including the requesting, reviewing and escalating of blood results.
2. Supporting the preparation of prescriptions.
3. Maintaining patient electronic databases and using digital clinical systems to review and update patients' digital records.
4. Making referrals to the rheumatology day unit, GP surgery, and other departments.
5. Liaising with homecare pharmacy team, third party delivery companies, and primary care.
6. Contacting patients by telephone to discuss treatment plans.
7. Identifying patients and running a nurse-led injection clinic for osteoporosis patients.
8. Attending clinics face-to-face or virtually with the biologic and osteoporosis nurses.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
