Lincolnshire Community Health Service NHS Trust
Lincolnshire Community Heath Service (LCHS) has an opportunity for a Registered Nurse to join their Wolds and Louth Community Nursing Team.
We have a full-time vacancy for Band 5 Community Staff Nurse.
As a community staff nurse, you will be required to work autonomously and as part of a team, delivering care to patients in their own homes, care homes, and clinic settings. Successful candidates will be responsible for assessing care needs, and the planning, delivery, and evaluation of nursing care using innovative evidence-based practice.
Main duties of the job
Community nursing is exceptionally rewarding as it allows wonderful therapeutic relationships to be developed with patients in their own homes. Working in this capacity embodies the person-centred care approach and allows patients to truly be at the centre of care delivery.
The skillset of a community nurse is vast and non-exhaustive, requiring knowledge and expertise in a range of areas. Examples of interventions provided are wound management, continence care, palliative care, medication administration, and long-term condition management. Close working with the MDT (Social Workers, GPs, and Allied Health Professionals) to ensure that patients' care is effectively coordinated demonstrates a positive impact on our patients.
Job responsibilities
As this is a community post, you must be able to travel to patients' homes and clinic settings. Due to the rurality of Lincolnshire, some areas are not well covered by public transport; therefore, having access to transport is essential.
The service works through the hours of 8:00 am to 6:30 pm, 7 days a week, and is covered by variable shift patterns of early shifts (8:00 am - 4:00 pm) and late shifts (10:30 am - 6:30 pm).
The Community Nursing service delivers a 12-week induction programme for all new staff. We also offer a robust preceptorship and clinical skills programme with an experienced clinical education team. This provides peer support in addition to the requisite training required to fulfil the role to its true potential.
You should use your application form to show that you meet all or most of the essential criteria outlined in the person specification for the position. If you fail to demonstrate this in your answers, or you are unable to meet the criteria, it is unlikely you will be shortlisted for an interview for that post.
