The Advanced Clinical Practitioner will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, acting within their professional boundaries.
The role includes:
* providing care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care
* demonstrate safe, clinical decision making and expert care, including assessment and diagnostic skills, for patients within the practice
* demonstrate critical thinking in the clinical decision making process and work autonomously within their scope of experience and qualifications but will have the support of the practice

Advanced Clinical Practice
* Provide holistic, high quality, autonomous, person-centred clinical care working at all times in accordance with NMC / GPhC / other professional guidelines and scope of competence
* Using expert history-taking, physical examination and clinical reasoning skills, make an assessment of patients’ physical, psychological and social needs and develop diagnostic and treatment plans, including appropriate prescribing, discharge, follow up or referral of patients
* Assess and manage patients with acute or chronic illness independently and provide advice and support for patients with self-limiting conditions in core daily clinics as appropriate to your scope of practice
* Understand and be able to manage patients presenting with multiple health conditions and complexity
* Exercise professional judgement to manage risk and escalate medical emergencies appropriately
* Work collaboratively across professional roles, boundaries, agencies and within teams
* Safely prescribe therapeutic medications appropriate to scope of practice
* Provide medication review services with patients in the practice including domiciliary visits to the local nursing homes and housebound patients, reconciliation of medicines following patients discharge from hospital or clinic appointments, medication reviews for newly registered patients and re-authorise medication for repeat prescribing within scope of practice
* Ensure contemporaneous, accurate documentation of all care given to patients
* Undertake clinical supervision in line with professional and CQC requirements
* To provide subject matter expertise on a specialist area of practice according to qualifications e.g. medication monitoring, emergency acute care, women’s health, palliative care etc
Leadership and Management
* Attend and contribute to monthly ACP meetings and ongoing ACP role development within the practice
* Provide clinical advice and support to other professionals within the practice
* Demonstrate a collaborative approach to team working within the practice and with outside agencies
* Promote mechanisms such as complaints/compliments, significant events & performance management processes in order to improve patient care and promote good practice
* Attend and input to the regular practice meetings
* Encourage cost-effective prescribing within the practice
* Review the latest guidance ensuring the practice conforms to NICE, CQC etc
* Support other clinicians with clinical matters, establishing positive working relationships
Research and Service Development
* Demonstrate an active role in the continuing development of the practice and support practice management plans, QoF and Enhanced Service targets via engagement in research/quality improvement activities
* Be able to use EMIS clinical system for data capture and to produce reports/statistics
* Assist in formulating practice protocols/guidelines as required
* Partake in audit as directed by the Clinical Services Team
* Support development of clinical governance systems as required
Education
* Critically assess and address own learning needs via appraisal processes and Personal Development Plan
* Attend regular training/updates as identified and agreed within your Personal Development Plan
* Maintain skills, and develop competence in new areas, depending on service needs
* Ensure professional knowledge is kept up-to-date
* Complete mandatory training
* Provide education on specialist area for the clinical team
Person Specification
Experience
* Experience of working with Primary Care
* Broad knowledge of General Practice
* Minimum of two years working at ACP level
Qualifications
* Professional Registration (NMC, GPhC, College of Paramedics)
* Advanced Clinical Practice at Masters Level
