The post holder undertakes technical duties in two or more of the following areas of pharmaceutical care:
1. Dispensary
2. Aseptic Dispensing
The post holder will be an integral part of the Pharmacy team working alongside all Pharmacy staff and link with a range of healthcare professionals to optimise concordance and compliance with prescribed medication.
Main duties of the job
Work closely with the Pharmacy Team and ward staff to ensure patients are able to derive the maximum benefit from prescribed medication.
Assess compliance with medication and any other factors related to medication which may lead to readmission.
Obtain accurate patient medication histories, using the appropriate sources, including GP surgeries, which will facilitate medicines reconciliation.
Provide skilled pharmaceutical counselling to patients in order to improve concordance and compliance with medicines and to reduce risks from medication-related events.
Work in partnership with the patient, carers, and relatives to reflect patient choice and support self-care.
Check patient's own medicines on admission for suitability for use within the Trust and alert the ward pharmacist of any discrepancies between this medication and the prescription chart.
1. Arrange supplies of newly prescribed medication.
Assist discharge planning through locker checks and supply of medication.
Liaise with community pharmacies and G.P.s if patients use Monitored Dosage Systems.
Ensure that patients are transferred to other wards with appropriate supplies of medication and any relevant information to the appropriate ward pharmacist.
Ensure that adequate levels of stock medicines are available on the wards.
Advise on the safe and secure storage of medicines at ward level.

Job responsibilities
Dispensary Duties:
1. Carry out the dispensing and issue of prescriptions in the Pharmacy of the base hospital.
2. Query prescriptions with prescribers, within the capabilities of a Band 4 technician under the supervision of a high-level Pharmacy Technician, or when delegated to do so by a pharmacist.
3. Contribute to the efficient use of the drug budget through effective medicines management whilst dispensing.
4. Process controlled drug orders and prescriptions.
5. Participate in the ordering, stock checks, and stock control of medicines in each area of the pharmacy, ensuring efficient use of pharmaceuticals and resources.
6. Follow guidance on rotas and prioritise own workload accordingly, in conjunction with the Senior Technician of the section.
7. Accurately input data, patient, and prescription details onto the computer system and assist with the maintenance of records relating to work and workload statistics.
8. Assist in the assessment of situations, some of which may require problem-solving skills, informing the relevant higher-level Pharmacy Technician if necessary.
9. Work toward achieving Accredited Checking Technician status following the in-house and regional framework.
10. May be required to undertake activities in relation to clinical trials in line with standing operating procedures.
Aseptic Duties:
1. Undertake the aseptic dispensing process according to SOPs once a required level of competence and validation has been achieved.
2. Maintain concentration at all times to minimize the risk of medication errors.
3. Work in a busy aseptic unit environment with frequent interruptions.
4. Spend a large proportion of the day standing or working at a computer.
5. Work in an environment handling hazardous compounds such as cytotoxic medicines, monoclonal antibodies, radiopharmaceuticals, industrial ethanol, and other cleaning products, and handling sharps.
Person Specification
QUALIFICATIONS / TRAINING
* BTEC/NVQ level 3 in Pharmacy services, including underpinning knowledge, or other equivalent qualification recognised by the General Pharmaceutical Council.
* Registered Pharmacy Technician with the General Pharmaceutical Council.

* Competent to work in Dispensary and on Wards.
* Demonstrate commitment to CPD.
* Knowledge of dispensing standards and legislation relating to hospital pharmacy.
* Calm and professional manner under pressure.

* Self-motivated.
* Meticulous attention to detail and able to prioritise workload.
* Aseptic dispensing skills, computer literate, Ward-based skills.

* Able to work on own or as part of a team.

Salary
£26,530 to £29,114 per annum, pro rata.
