As a Care Coordinator, you will play a vital role in ensuring that patients receive the support they need to achieve optimal health outcomes. This position requires excellent communication skills, strong organisational abilities, and a compassionate approach to patient care.
The post holder will deliver core patient pathway duties to our practices as one of the multidisciplinary team. Duties will include dealing with patient and clinical queries/ requests, processing discharge summaries/clinic letters, undertaking clinical audits and promoting best practice within the surgeries where they are based. The post holder will work to assist in the delivery of an all-encompassing approach to personalised care and promoting and embedding the personalised care approach across the PCN at a neighbourhood population level.
This role would be ideal for an experienced medical receptionist or NHS data administrator that is passionate about patients and the care they receive. This role is partly patient facing.

Essential Criteria:
* NVQ Level 2/ 3 qualification in a relevant field (Health and Social care or customer service or other relevant qualification) OR
* Experience of working within healthcare, the voluntary or community sectors, or patient related services
* Applicants must have GCSE Maths, English and Science at C or above (or equivalent)
* Prepared to enrol in, or undertake appropriate training as set out by the Personalised Care Institute
* Excellent IT and patient/ customer service skills
* Driving licence and transport
Desirable Criteria:
* Experience within a primary care working environment
* Experienced clinical coder

This role will be based mainly at our Gunnislake Branch, although some working at Callington is required.
Hours:
36 Hours a week over 4 days between Monday to Friday (9 hour days). Between the hours of 8am-8pm, or 37.5 hours a week over 5 days Monday - Friday (7.5 hour days).
Salary Range:
The starting salary for this post is between £12.20-£13.08 per hour (dependent on completion of the care coordination course).
The ability to study to at least a level 2 ability is a requirement of the role.
