Job summary The post holder will be based at Monnow Vale Health & Social Care Facility, Integrated Service Hub for Central Monmouthshire, workingalongside health & social care colleagues in a well-established community based Integrated Team. The post holder will lead on thedevelopment & delivery of a specialist falls service for Tier 3 Complex Fallers, engaging the wider MDT, and working alongside existingqualified and support staff, and support delivery of a physiotherapy service to Tier 2 Fallers requiring Reablement input.The postholder will additionally manage a general community-based caseload as required The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply. Main duties of the job Provide advanced clinical falls risk assessment, clinical reasoning and diagnostic skills, for older people with a wide variety of complexfalls presentations for service users with unexplained falls, dizziness and giddiness in a variety of community settings, including HMPUsko Devise bespoke intervention management and rehabilitation programmes for this specialist caseload based upon the findings of thespecialist assessment.
