Job summary To provide Clinical Leadership and first line management to the Looked After Children's health team for the purpose of meeting the health needs of children in care, as recommended in the Department of Health Statutory Guidance 'Promoting the Health and Wellbeing of Looked After Children' (DoH May 2002). This post involves active multi-agency working on behalf of looked after children, and the ability to develop a service for a specific client group. This role will require the post holder to build effective working relationships with the medical team conducting Initial Health Assessments, the designated safeguarding children team as well as the operational management team and admin service. Main duties of the job 1. To model best practice in health assessment and care planning for looked after children, including creating and implementing care plans and promoting access to services. 2. Ensure that the health of Looked After Children is positively promoted, encouraged and supported. Empower children and young people to take responsibility for their own health and wellbeing 3. Set up and maintain health promoting education activities for individuals and groups of children and young people, carers and professional staff using evidence-based interventions 4. To lead the clinical team in building the face-to-face service model of care for looked after children 5. To provide specialist advice and consultation to other professionals, volunteers, parents and carers in order to facilitate good health in this client group 6. To be proactive in using research and evidence-based interventions, and in consulting with looked after children and in the planning of services 7. To work with the Specialist Nurses and Doctor for looked after children to design services to meet the needs of the client group 8. Undertake Duty cover for the service on a rota basis. This includes attendance at Strategy meetings as they occur 9. Complete one Review Health Assessment (RHA) per week to support the performance model for the service ensuring that Children and Young People (CYP) receive their RHA within the 12 month timeframe.
