We seek a Band 6 Occupational Therapist to join our friendly team on a full-time basis. You’ll be making a real difference to your patients every single day - delivering outstanding community care as a valued member of the Discharge Home to Assess Team.
A full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).
Main duties of the job
The Discharge Home To Assess service runs 8 am to 6 pm Monday to Friday and 9 am to 5 pm at weekends. The post holder will be expected to contribute to the weekend working rota.
This post will close early if we reach the maximum amount of applications; early application is encouraged.

Detailed job description and main responsibilities
* To provide therapy to patients within their own home.
* To provide assessment and treatment for vulnerable patients with complex needs, patients in crisis, supported discharge from either the acute hospitals or those in intermediate care bed-based units within the community.
* To act as the patient’s advocate throughout their rehabilitation programme and enable patient empowerment through the process.
* To communicate complex patient-related information effectively and ensure collaborative working with the multidisciplinary team members across health and social care sectors.
* To maintain accurate and up-to-date documentation in line with legal requirements and communicate assessment and treatment results to the appropriate disciplines.
Person specification
Qualifications/Education
* Occupational Therapy Degree
* Experience working within the NHS
* Good communication skills and experience with complex patients
* Registration with the Health Professions Council
* Post-graduate courses
* Relevant courses in Long Term Conditions Management
* Experience providing supervision
* Experience working within the Community
Experience & Knowledge
* Experience of working in the community.
* Good communication skills and working with complex patients.
Skills and Abilities
* Communicating within a multi-discipline team
* Able to communicate where there may be barriers to understanding
* Excellent oral and written communication skills
