The Community Mental Health Team for Older People is busy, requiring a timely response to requests for services. However, you will be given time to meet the demands of the service as part of a supportive team. You will have the opportunity to reflect and solve problems and develop practice.
The Role:
* You have a key role in identifying vulnerable people with needs to support them living their life with least restrictive support and providing direct work to their family members.
* You will be a member within a wider multi-disciplinary team; being an active voice of the social care principles during MDT meetings, reflective learning sessions and complex case forums.
* To enable this, you are expected to work autonomously but also work as a team member.
* To hold outcome focused conversations with residents to assist them in finding solutions to the issues that have prompted them to contact us.
* To provide assessments, when required, to eligible residents within the Royal Borough and facilitate the delivery of those identified services in order to achieve desired outcomes.
* To actively engage with the third sector and be able to confidently signpost residents to appropriate resources within the community.
* To complete holistic and person-centred reviews of the assistance and support that is provided to our residents.
What we are looking for:
* A recognised Social Work qualification and current registration with Social Work England.
* In-depth knowledge of relevant legislation and policies, including the Care Act, Mental Capacity Act, and safeguarding processes.
* Strong leadership skills with the ability to supervise and support team members and students.
* The confidence to represent RBWM in multi-agency settings and make sound, person-centred decisions.
* A proactive, flexible approach to managing change and driving service improvement.
