We are recruiting for a full-time Band 6 CPN to join our team, working within the Denbighshire Older Persons Community Mental Health Team.
Main duties of the job
As Care Coordinator, you will hold a caseload of patients with organic and functional mental health needs, travelling to see them in their own homes, in care homes, and in-patient settings.
If you are an experienced Mental Health Nurse with passion and enthusiasm for working with older adults, then we would love to hear from you.

Job responsibilities
1. As Care Coordinator, ensure that Care and Treatment Plans and Care Management Protocols are adhered to and that standards are met in line with the Mental Health (Wales) Measure 2010.
2. Be part of the Duty Nurse rota, covering both North and South Denbighshire for our service - undertaking first line mental health assessments, risk assessments, and supporting Mental Health Act assessments as required.
3. Undertake supervision of Band 5 CPNs, HCSWs, and Student Nurses as required.
4. Administer Depot medication as required.
5. Autonomous and joint working as part of a multi-disciplinary team.
