Job summary The post holder will be part of a team dedicated to ensuring patient experience and engagement informs the care provided by the Trust. The post holder will act within their professional boundaries to support people with long term health conditions or cancer to develop the knowledge, skills, and confidence to manage their health and wellbeing. The role will assist to increase patient ability to access and utilise community support whilst providing access to self-management, education and peer support. It is key within this role to work closely with the Trust's multidisciplinary teams, as well as our patients, relatives and carers and with the wider community, to ensure we are hearing what our community have to say and work together to achieve the best possible patient experience. A key part of the role will be to promote awareness days, by networking with our Clinical Nurse Specialists across the Trust, to facilitate events and lead on support groups for the trust and the local community. Main duties of the job Establish network relationships with existing Cancer Support Workers, Health Coaches, and other relevant health professionals, within Cancer Services and the wider healthcare community including partners in primary care, community services and mental health services, to ensure wrap around care of the individual. Provide regular support to patients, relatives and carers with long term health condition(s) including cancer, assessing their holistic needs, and supporting them to self-manage by providing the relevant information and advice and signposting them to other resources and services available to meet their identified needs. Visit patients who have been in hospital for over 4 weeks, to assess their health and well being and provide support/advice as required. Following all clinical input, coordinate care with the person living with long term condition(s) so they can access the right help at the right time, providing a seamless experience of care. Check that the person is able to self-manage and provide a flexible and personalised response. Support those being treated for or living with long term condition(s) including cancer, to navigate the complex health and social care system, by building relationships with professionals and facilitating access to these services, including generating referrals on the person's behalf where necessary.
