This is an exciting opportunity for an Exercise Practitioner to join the Active Together Cancer Rehabilitation service in Sheffield.
Active Together is a prehabilitation and rehabilitation service supporting people following a cancer diagnosis with physical activity, nutrition, and wellbeing guidance.
The service has been designed by Sheffield Hallam University who work in close collaboration with Sheffield Teaching Hospitals NHS Foundation Trust and local NHS trusts across South Yorkshire to support the delivery and evaluation of the service.
Active Together is funded by Yorkshire Cancer Research, Yorkshire's independent cancer charity, working so more people can live longer, healthier lives, free from cancer.
We are seeking a highly motivated person to join our innovative and collaborative cancer rehabilitation service to help deliver and develop the service for cancer patients in Sheffield. Active Together Sheffield is based at the Advanced Wellbeing and Research Centre on the Sheffield Olympic Legacy Park and is also delivered at several leisure centre locations across the city.
Main duties of the job
1. To deliver physical activity prehabilitation and rehabilitation programmes to patients undergoing a range of oncology treatment pathways in both one-to-one and group settings at sites across the city.
2. To assess the progress of patients during their time with Active Together and with support adapt their programmes appropriately to ensure safe and effective prehabilitation and rehabilitation.
3. To promote healthy living in patients and support behaviour change.
4. Adopt a personalised care approach to the management of patients, including an understanding of behaviour change approaches and "what matters to you".
5. Contribute to the evaluation of the service against Key Performance Indicators (KPIs) for feedback to Yorkshire Cancer Research.
6. Understand the need to address and reduce health inequalities and promote health inclusion within Active Together as a priority in patient care and service development.
7. Contribute to operational management, leadership and organisation to ensure continuous quality improvement of the Active Together Sheffield service.
8. Demonstrate an awareness of wider Active Together delivery across South Yorkshire and contribute to service development opportunities with support as required.
About us
This vacancy is advertised on a fixed term contract basis for a period of 11 months due to the post being externally funded. The fixed term contract status of the post holder will be reviewed in accordance with the Trust's legal obligations regarding fixed term contracts taking into consideration the ongoing source of the external funding and the requirement for the post to be maintained.

The Active Together Programme is a collaboration between Yorkshire Cancer Research, Sheffield Hallam University and Sheffield Teaching Hospitals which provides a research-based prehabilitation and rehabilitation service to patients as part of their cancer treatment pathway.
The service is multimodal with a focus on physical activity, nutrition and mental health, with a multidisciplinary team including, Physiotherapists, Exercise Specialists, Dietitians, Psychologist, Fatigue Specialist and Occupational Therapist.

Person Specification
Education, Qualifications and Memberships
* Level 3 Diploma in exercise referral (CIMSPA accredited) or a degree in sports science or similar
* Level 4 Diploma in exercise referral (CIMSPA accredited) or a degree in sports science or similar Application form Level 4 Cancer Rehabilitation or willingness to work towards this
* Demonstrate commitment to Continuous Professional Development via personal development plans/file
Experience and knowledge
* Experience of carrying out 1 to 1 and group exercise programmes with a clinical population
* Experience of clinical assessments, problem identification and guided treatment
* Knowledge of health-related physical activity
* Knowledge and understanding of goal setting principles
* Knowledge of behaviour change methods
* Experience of working with people with cancer.
* Experience in delivering community based rehabilitation
* Experience in supporting delivery of cardiac, pulmonary or cancer rehabilitation
* Knowledge and awareness of health inequalities.
Skills and Qualities
* Ability to work independently as an autonomous practitioner when required.
* Excellent organisational and time management skills to manage a demanding workload and priorities effectively
* Experience of working collaboratively as part of a multidisciplinary team.
