Job Overview
The Mental Health Recovery Team/CMHT is looking to recruit a Band 6 Mental Health Transition Practitioner to support transition for young adults aged 18-25 into the CMHT/Recovery Team and connect them to their chosen pathway and interventions.
The successful applicant will engage the young adult, carer, and referrer prior to referral, where possible, supporting the young adult to make informed decisions around their treatment plan and transition.
Main Duties of the Job
The Transition Practitioner will work within the CMHT/Recovery, supporting young adults for a limited time until they achieve their transition goals and begin treatment within the team. They will act as their keyworker, helping the young adult navigate the mental health system.
The Transition Practitioner will regularly analyze performance against transition standards to identify areas for quality improvement.

Detailed Job Description and Main Responsibilities
The Community Mental Health Transition Practitioner will support transition for Children and Young People aged 18-25 from Mersey Care Children and Young Persons Services (CYPS) to Adult Community Mental Health Services.
Principal Responsibilities:
1. Oversee implementation of NICE Quality Statements for transition.
2. Ensure a coordinated transition plan for young people moving to adult services.
3. Facilitate annual meetings to review transition planning.
4. Coordinate care and support during transition.
5. Ensure person-centered and strength-based transition support.
6. Collaborate with health and social care services for smooth transitions.
Person Specification
Qualifications
* Degree level training or equivalent.
* Professional registration (NMC, Social Work England, etc.).
* At least one year of post-registration experience.
* Evidence of CPD and working towards a master’s level qualification.
Knowledge and Experience
* Experience in mental health care within a secondary care environment.
* Experience of working as a care co-ordinator/key worker.
* Experience in developing collaborative care plans.
* Knowledge of the Mental Health Act and Mental Capacity Act.
Skills
* Ability to work independently and as part of a team.
* Ability to engage service users and develop transition plans.
* Ability to problem-solve and implement risk management strategies.
