Make a difference to the lives of pre-school children with significant additional needs
Hampshire County Council’s Services for Young Children (SfYC) Inclusion Team is dedicated to improving outcomes for children with Special Educational Needs and Disabilities (SEND). We work in partnership with educational settings, families, and other agencies to ensure that children with SEND receive the support they need to thrive.
We are seeking a dedicated and experienced Transition Practitioner to join our team. This role is part of a project aimed at supporting children with SEND who are transitioning into Year R without an Education, Health and Care Plan (EHCP). The goal is to provide early interventions and services to prevent the escalation of needs and ensure a smooth transition into mainstream school. Supporting children to have their needs met in a fully inclusive environment.
If you’re considering this role, here are some key qualities and skills that would be beneficial:
* In-depth knowledge of child development: Understanding the early years of child development is crucial, especially for children with SEND.
* Experience with families and pre-school children: Practical experience in this area will help you connect and support families effectively.
* Organisational skills: Being well-organised will help manage the various aspects of the role efficiently.
* Self-motivation: This role requires a proactive approach to meet the needs of the children and their families.
* Communication skills: Being a good listener and communicator is essential to understand and address the concerns of families.
* Tact and sensitivity: A sensitive approach is necessary to handle the unique challenges faced by families.
If you are passionate about supporting children with SEND to have a successful start in school and have the skills and experience we are looking for, we would love to hear from you.
