Job summary This is a Band 6 community mental health nurse role working within Older adult services in Bury. This role involves responsibilty for providing interventions to identified clients in a community setting, involving severe and enduring mental health issues and particularly challenging and complex cases. Main duties of the job To work in the community and liaise with in-patient areas assessing andprioritising clients appropriately.To manage a caseload of clients with complex and challenging mental health needs.To assess clients in the most appropriate location often in their own homeworking in accordance with the Lone working Policy.To plan and implement appropriate client centred interventions to meet the needs of the individual. Contribute to making decisions regarding client care in consultation with themulti-disciplinary team.Undertake the assessment, planning, implementation and evaluation of of care.Maintain standards with regards to the quality of interventions provided.
