Job summary Join our Hospital Social Work Team as a Senior Practitioner where you will be influential in driving positive change and leading the team through this exciting journey and a period of organisational development. As an experienced Social Worker, you will operate with a high degree of autonomy and authority using a range of evidence-based, social work interventions and will hold a more challenging and complex casework. Alongside this you will provide professional support to the Hospital Social Work Team. Main duties of the job It is widely known that the majority of patients recover better at home, both physically and mentally. Acknowledging this, Manx Care is working hard to change the balance of care from hospital to the community and streamline the discharge of patients from hospital as soon as is appropriate. We aim to develop a Hospital Social Work Team that works alongside ward based Multidisciplinary Teams, as well as our Complex Discharge and Access and Capacity Teams, to streamline transfer of care and discharge pathways. This integrated team will lead discharge planning for those patients with complex needs who require coordinated care and support to return home.
